New Patients

In order to provide you with the best possible care and to prepare for your dental visit with us, we ask that you complete all necessary forms prior to your initial visit with us. Once we create your appointment in our system, it will generate a confirmation via text (or email). In addition, all necessary paperwork links will be forwarded to you. If you are not tech savvy, we are happy to assist you in the office with completing the digital forms. Please call our office if you require assistance.

We want to meet your dental needs, and the completion of these forms will assist us in making your dental visit comfortable and safe. Our intake forms may seem a bit lengthy, but please take your time answering all questions to the best of your ability with as much information as possible. Remember; your dental health and whole body health go hand in hand. We will utilize your dental and medical health history in determining the best treatment route for you as an individual. Your thorough completions of our intake forms also assists us in determining if your treatment(s) qualifies for medical insurance coverage. If you have any questions at all, or need assistance with completing any forms, please reach out to us prior to your appointed date and time.
